Errors in care can be caused by human error and sometimes equipment failures – with human error being responsible for about 60 – 70% of errors.
As the saying goes ....to err is human. We make mistakes, however in care the consequences of these mistakes can be catastrophic. Human Factors aims to acknowledge this fact and build safer systems knowing that we are fallible.
